Abstract

The impact of the COVID-19 pandemic has made the coordination of health and social systems urgent and has put the focus on new risks and needs. Social and health coordination measures should ensure quality care in residential centers and ensure that residents retain and combine their rights to health care from the National Health System with those derived from their dependency status, with the quality standards of both systems. Among the conditions for effective coordination of health and social services, it is necessary to highlight: A decisive technological and organizational investment, a long-term vision, a framework of guaranteed rights in social and health matters, public responsibility with private participation. For the cohesion of social and health care, it is essential to define the profiles of the social and health population, establishing a single information and access procedure that is common at the state level, defining the composition of the case management team and a single and shared social and health history. The introduction of the Single Digital Social History (HSUE) has meant a commitment to digital management, as well as an important advance and innovation in the field of Social Services in Andalusia.
